In dieser retrospektiven Single-Middle-Studie haben wir die Ergebnisse der Lymphozyten- und Neutrophilenzahlen von 1450 iranischen Patienten mit Coronavirus-Krankheit 2019 (COVID-19) überprüft, die im Baqiyatallah Hospital, Teheran, Iran, rekrutiert wurden. Von 1450 Patienten waren 439 Fälle (30,3% ) negativ für die Polymerasekettenreaktion (PCR);; Hervorheben, dass unfavorable molekulare Exams nicht so zuverlässig sind wie constructive Ergebnisse.

Während die Lymphozytenzahl in Fällen mit weniger als 50 Jahren 1,8 × 103 / µL (1,2-2,5) betrug, betrug sie in der älteren Gruppe 1,47 × 103 / µL (0,84-2,16) (p <0,001). Auch Männer hatten im Vergleich zu Frauen niedrigere Lymphozyten (1,53 × 103 / & mgr; l gegenüber 1,76 × 103 / & mgr; l; p = 0,002). Von besonderem Interesse conflict, dass die Lymphozytenzahl in den PCR-negativen Fällen 1,77 × 10 3 / & mgr; l (0,98–2,45) betrug, was signifikant höher conflict als die Anzahl in ihren positiven Gegenstücken (1,53 × 10 3 / & mgr; l; p = 0,004).

Im Gegensatz zu Lymphozyten hatten Geschlecht und PCR keinen signifikanten Einfluss auf die Anzahl der Neutrophilen. Das Odds Ratio für Neutrophilie bei Patienten über 50 Jahren, entweder mit negativer oder positiver PCR, betrug 2,46 und 2,23, was darauf hindeutet, dass das Alter der wichtigste assoziierte Faktor ist.

Die Anzahl der Lymphozyten zusammen mit einer erhöhten Neutrophilenzahl kann wahrscheinlich als einfacher, schneller und wirtschaftlicher Biomarker dienen und scheint ein geeigneter Punkt zu sein, der bei der Identifizierung von Patienten mit COVID-19 berücksichtigt werden sollte, insbesondere von Patienten über 50 Jahren.

Hier identifizieren wir eine G-zu-U-Transversion (Nucleotid 26372)im SARS-CoV-2 E-Gen in drei Proben mit verringerter Virusnachweiseffizienz unter Verwendung eines weit verbreiteten kommerziellen Assays. Eine weitere Analyse des öffentlichen GISAID-Repositorys führte zur Identifizierung von 18 zusätzlichen Genomen mit dieser Mutation, die fünf unabhängige Mutationsereignisse widerspiegeln. Diese Arbeit unterstützt die Verwendung von Twin-Goal-Assays, um die Anzahl falsch negativer PCR-Ergebnisse zu reduzieren.

Hämosporidische Infektionen bei Haushühnern (Gallus gallus domesticus) sind nicht nur weit verbreitet, sondern verursachen auch wirtschaftliche Verluste. Die Diagnose wird normalerweise durch mikroskopische Untersuchung gestellt; Das Verfahren weist jedoch mehrere Nachteile auf, wie beispielsweise das Erfordernis eines erfahrenen Mikroskopikers, das unzuverlässig ist, wenn die Parasitämie gering ist, und nicht in der Lage ist, zwischen Koinfektionen von mehreren Parasitenarten genau zu unterscheiden. Daher könnte das derzeitige Ausmaß der hämosporidischen Infektionen unterschätzt und vernachlässigt werden.

Wir haben einen neuartigen Multiplex-PCR-Assay entwickelt, mit dem vier hämosporidische Parasiten gleichzeitig nachgewiesen und unterschieden werden können: Leucocytozoon caulleryi, Leucocytozoon sabrazesi, Plasmodium juxtanucleare und Plasmodium gallinaceum. Die Primer in der vorliegenden Studie amplifizierten spezifisch die entsprechenden Ziele, ohne dass eine speziesübergreifende Amplifikation nachgewiesen wurde.

Die Multiplex-PCR zeigte im Vergleich zur mikroskopischen Untersuchung eine signifikant höhere Nachweisrate (p = 0,0001) . Die Ergebnisse zeigen, dass die Nachweisrate der Multiplex-PCR für L. sabrazesi, P. juxtanucleare und P. gallinaceum alle höher ist als die der mikroskopischen Untersuchung mit p = 0,002, 0,0001 bzw. 0,004. Koinfektionen wurden auch durch Multiplex-PCR wirksamer nachgewiesen.
